- All new patients
- Patients seen by Dr. Siegler in hospitals but not yet established in the office.
- Patients seen by Dr. Siegler in the office but:
- Have not been seen by Dr. Siegler for an extended period.
- Have a new problem different than that seen by Dr. Siegler in the past.
